Genetic results are not a verdict. A negative result does not erase family history, and a positive result does not mean a cardiac event is inevitable. What it does provide is a more informed risk framework for treatment decisions, family screening, and long-term monitoring.

Coronary artery calcium scoring

A coronary artery calcium scan, often called a CAC scan or calcium score, is a fast, noninvasive CT scan that measures calcified plaque in the coronary arteries. It does not require contrast and typically involves a relatively low radiation dose.

For the right patient, a calcium score can change an abstract discussion about risk into a specific prevention decision. A score of zero can be reassuring, particularly in someone without major high-risk conditions. But it is not a lifetime clearance certificate. It does not detect noncalcified plaque, and its meaning depends on age, symptoms, smoking status, diabetes, LDL level, and family history.

Any calcium score above zero confirms that coronary atherosclerosis is present. The amount and distribution of calcium can help guide the urgency of lipid lowering, blood pressure management, lifestyle intervention, and follow-up. A higher score is not a reason for panic. Coronary CT angiography

Coronary CT angiography, or CCTA, provides a more detailed view than a calcium score. Using CT imaging and intravenous contrast, it can visualize both calcified and noncalcified plaque, assess coronary artery anatomy, and identify narrowing that may need further evaluation.

CCTA is not a routine screening test for everyone. It involves contrast, more radiation than a CAC scan, and a more involved preparation process. It is most useful when clinical questions remain after risk assessment, when there is concern for premature plaque, or when a clinician needs a clearer picture of coronary anatomy. The decision should be individualized rather than driven by curiosity alone.

How to choose the right sequence of tests

The most efficient approach is usually staged, not scattershot. Start with a preventive cardiology consultation and a careful review of personal history, family history, medications, prior labs, blood pressure, exercise tolerance, and goals. Advanced bloodwork often establishes the biochemical foundation.

Testing is only the beginning. The real value of preventive cardiology is translating results into a clear, sustainable strategy. Depending on the findings, that may include nutrition and exercise recommendations, weight and sleep interventions, blood pressure optimization, targeted medication discussions, repeat biomarker testing, and periodic imaging when clinically appropriate.

For some patients, lifestyle change is the central intervention. For others, especially those with established plaque, severe inherited risk, or substantially elevated atherogenic particles, medication may be an essential part of prevention. This is not a failure of discipline. It is a rational response to measurable biology.

A strong plan also defines follow-up. Patients should know which numbers matter, what their treatment targets are, when they will be reassessed, and what would trigger a change in strategy. Vague reassurance is not a prevention plan.

Preventive testing is not designed to create anxiety or turn every healthy person into a patient. It is designed to replace uncertainty with clinically useful clarity.
